# Decision records

No product/design decision records have been created yet.

Add records under `Records/` with a unique defining `decision_id`, status,
decision, rationale, source/approval evidence, consequences and revisit trigger
when temporary. Record actual accepted decisions; do not fill this index with
invented examples. Link each record here and keep [Active](Active.md) concise.
