feat: Akinator 2.0 - every prompt documented, the repository is its own wiki

Inside the one skill and the one command:
- akinator-wiki station: every prompt fans out to every home it affects
  (product, business, market, requirements, drift, architecture, libraries,
  stack, infra, testing, UX, project, decisions, changes, glossary, onboarding)
- akinator-decide station: decide the reversible, recommend the rest with
  costed options
- extract_libraries.py (a generated page per dependency, curated why kept)
  and akinator_wiki.py (init, index, gaps, check)
- ledger gains requirement (missing/changed/current/dropped) and drift records;
  the brief ranks them first
- question budget 15, grouped, each with a recommended default
- honest gap marker instead of invented facts
- rule 13, ADR 0010, CI drift checks for library pages and the wiki index,
  templates with filled examples, living wiki filled for this repo
