Never add or rename a column.\n\n**Row-count invariant — check before emitting.** EXACTLY ONE row per plan code that priced successfully (drop failed codes; never fabricate). If there are more, rebuild with one row per code. SEMH appears once, at `$0 deductible`; STCIH once, at `Unlimited coverage`.
