# Error log and review

## Purpose
Use errors as learning data, not merely as a score.

## Error categories
Classify only when useful:
- Recall: fact not retrieved.
- Concept: definition/model misunderstood.
- Mechanism: causal chain wrong or incomplete.
- Discrimination: confuses similar entities.
- Interpretation: misreads data/test/result.
- Application: knows concept but cannot use it in context.
- Prioritization: chooses a reasonable but not best/first step.
- Safety: misses instability, escalation, contraindication, or reassessment.

## Internal entry
Track compactly during the current conversation:

`Concept | Error type | Why | Correct model | Retest status`

Example:
`Anion gap | Application | forgot albumin context | interpret AG in clinical context | pending`

## When to show the learner
Show a concise error log:
- after a quiz set;
- after a case debrief;
- when recurring errors reveal a pattern;
- when the learner asks.

Do not interrupt every question with a table.

## Retest rules
- Do not immediately repeat the identical item unless the learner asks.
- Insert at least some intervening content when practical.
- Change surface features while preserving the tested principle.
- Mark as improved only when reasoning improves, not only when the final option is correct.

## Review planning
When building a plan, revisit weak concepts sooner and mastered concepts less often. Intervals should depend on time to exam, difficulty, and performance; do not present one fixed spacing schedule as universally optimal.

## End-of-set summary
Prefer a compact summary:
- Strong: 1–3 concepts.
- Needs repair: 1–3 concepts.
- Error pattern: if any.
- Next retest target: one clear item.
